🧭 Clarity Care Gold — £325/month This is a monthly membership (15 minutes is start up time) For families navigating more active or complex care situations, including discharges, early-stage concerns, or care reviews. Includes everything in Silver, plus: • One in-person or virtual support session (1 hour) per month • Care Plan & Risk Assessment Review once during the 3-month cycle • Priority response time (within 48 hours) • Access to discounted additional services (e.g. advocacy, mental capacity support) • Optional: I can attend one virtual or local care meeting on your behalf every 3 months 💡 Ideal for those needing practical help and guidance during transitions, early dementia stages, or care plan confusion.
